Insurance Investigations

My family started working in the insurance industry in 1971. Three years later I started my twenty-three year career in law enforcement. I worked as a New York State University Police Officer at Stony Brook,a California Correctional Officer at San Quentin, the San Francisco Police Department as a plain clothes investigator, a uniformed patrol officer and a LWLP Sergeant. I worked Under Cover Narcotics as a "Special Agent" for a (Mid West Narcotic Task Force called a MEG Unit), as a Sworn Deputy Sheriff in ten counties covering ten thousand square miles, and as a Chief of Police in South Dakota. I have since retired from law enforcement, while my two sisters became corporate regional Managers for two of the nation's largest insurance carriers. In 2008 one sister retired, and now helps FCI with our insurance investigations.

I have always said "surveillance is an art, you are either born with it or not". I conducted my first "surveillance" in 1974 as a bonded New York City Police Officer (NYPD). I was assigned to a "Robbery Suppression Team" and my first assignment was to conduct an armed two-man surveillance looking for two armed gunmen conducting daytime armed robberies at Macy's on 34th St. in New York City. The robbers were holding up Macy's Christmas shoppers at gun point.

Since 1974, I have graduated four major law enforcement academies, and countless training events by local, State and Federal Law Enforcement Agencies. I was told by the San Francisco District Attorney's Office that I was the highest federally trained police officer in S.F.P.D, from 1980 through 1995, with over 2000 hours of documented formal police training, most of it from the United States Drug Enforcement Administration, U. S. Department of Justice, and the Federal Bureau of Investigation.

  • AOE/COE: means "arising out of employment, or during the "course of employment" and requires investigators to gather all of the factual information and statements as they relate to an actual incident. GET THE FACTS, before they change!
  • SUB ROSA: Is Latin for "under the rose" where meetings were held in secrecy under a rose Centuries ago. The term is now used by insurance firms referring to the secrecy of surveillance of alleged injured workers, some of which are just plain fraudulent and some people are exaggerating their injuries. We video these claimants and furnish the film to our Clients. I have spent over 38 years doing surveillance for many clients and reasons. Many people do not realize, surveillance is a gift, you are either born with it or not. I am the best at "surveillance" and have spent most of my law enforcement career totally under cover with a different name living as someone else. With Insurance carriers, "Sub-Rosa" is their term to film/video a claimant regarding his daily ability to function or work. As mentioned above, Sub-Rosa is Latin for under the rose used centuries ago. Certain people would meet under this "rose" which in fact meant they were swearing their entire family to an oath of secrecy based on a penalty of death. Our percentage rate for getting much needed video evidence on exaggerated and fraudulent claims is extremely high. So if you have a case where you stand to lose thousands or millions of dollars and you know the claim is fraudulent or exaggerated, you need to call us. Remember, proper Surveillance is an art. The last thing you need is someone to attempt surveillance and get "BURNED". Meaning they were seen by the subject. FCI does not even take the chance. We use at least two investigators who utilize our own radio network for split second communication, which is a must if you are going to be successful.

This person claimed to be 100% disabled and unable to work. BUT, He was well enough to lift/carry a sixty pound battery and go fishing on his boat.

Since 1991, we have successfully investigated THOUSANDS of industrial and non-industrial Insurance cases. We furnish our clients with clear/concise reports, and professional quality DVD and VHS video evidence. Our clients are updated on a regular basis and their completed reports are sent via the Internet, hand delivered, and/or sent U.S. Mail. The client gets what he requires.

Our reports contain still color photographs, inserted in the actual report. These pictures are gleaned from the actual video surveillance film, depicting the claimant's identification and any highlights that might be of interest to the case. This allows the adjuster/client to visually follow along, while reviewing the report and film. Identification photos are also included. (See above)

A typical case starts with our own preliminary investigation. We obtain prior W/C records if needed, and we go over the documents given to us by the client to determine what, if anything, we need to add. Our field work consists of an activity check to determine the proper method of surveillance. Once this preliminary activity check is completed, we proceed with the actual investigation. We adhere to state law and have strict guidelines regarding personal privacy. We adhere to the NAIC's Model Act regarding personal privacy and surveillance investigations. By utilizing both methods of investigation and surveillance, we are able to provide a superior work product that is concise and accurate. Our surveillance film allows the Client to determine the extent of the injury, and whether there appears to be any fraud or exaggeration. If the claim is accepted, a major benefit of our surveillance film is that it usually shows the extent of the exaggeration, therefore, keeping the claim to a minimum.

We utilize state-of-the-art surveillance systems for acquiring both indoor and outdoor film, and our success rate is over 98 percent. We also utilize a satellite communications network that enables investigators to be in constant contact with adjusters via radiophone and/or the Internet. We service the investigative needs of TPAs, self-administered carriers, large and small employer's Human Resources and law firms.

I am a graduate and valedictorian of the United States Drug Enforcement Administration's Academy in Washington, DC, and have been conducting in-depth undercover investigations for over 38 years. Our video/film success rate, (98 percent) is the direct result of surveillance skills learned while working with the FBI and DEA. I have passed on everything I know to my son, Steven, who has been working these cases with me for many years.
